Hospital Forms
If you would like to "drop off" your pet instead of having an appointment (either you don't have time or there are no appointment available), feel free to download this form where you can explain your pet's condition, and provide contact numbers so we will be able to communicate with you once your pet has been examined.
To help make your visit to our hospital or lodge as easy and efficient as possible, you can download any of the following forms prior to your visit. Complete the form, as much as your computer allows, print it, and bring it in with you to save time when you come in. Please only complete those parts of the form that the computer permits - the rest of the form must be filled out when you bring your pet in. If you have any questions regarding any of these forms, please call or ask at time of check - in.
For our new clients this form needs to be filled out so we have contact information for you, and the names, ages, and other information regarding your pets.
If your dog is scheduled to have a surgical procedure performed, feel free to review, and even complete this form prior to the morning of your appointment. There are many options available for your pet, that while not required, may make the procedure more comfortable or even safer. We do not require these for every surgery, but we do make them available. This way we can keep the procedure as affordable as possible, while still providing the best quality medicine currently available.

If your pet is scheduled for a dental procedure, filling this form will explain to us what procedures you are willing to authorize, as well as contact numbers so we can contact you, if needed, during the procedure. Once your pet is anesthetised, a complete oral exam is done, and it is not uncommon that previously hidden problems are found. Letting us know how involved you wish us to get, and how to contact you if needed can save critical time while your pet is anesthetised.
Lodge Forms
This form needs to be completed so we know how you want us to take care of your dog(s) while you are away. Please do not fill out any of the form that is not highlighted in blue. Fill out the form on your computer, print it, and bring in it on the day of boarding check - in. The remainder of the form needs to be filled out by a staff member. Filling out any additional information by hand will void the form and a completely new one will need to be filled out at check - in.

This is the form you need to fill out, and bring in so we can keep it in your file if you might need someone else to give authorization to treat your pet. This is often used for pet-sitters or friends who are watching your pets while you are out of town.
